Trans-Care and Transportation Program

• We can provide a ‘Personal Support Worker’ (PSW) in the hospital and the home. We can also assist with the transition from hospital to home (or nursing home for seniors)
• Prescription pick-up, essential groceries, small medical equipment, or supplies as required
• Transportation with wheelchairs and other important equipment
